Shands Patient Information

Upon arriving at the Primary Care Clinic, first time patients can expect to spend approximately 20 minutes completing paperwork that will facilitate our staff as we meet your needs. Should patients wish to fill out the forms prior to their appointment, they are attached below for your convenience.

Office Visit

Please bring the following items with you to your first appointment:

• Previous medical records 
• Current insurance card (including Medicaid card) 
• List of allergies 
• Current medications and dosages 
• List of other physicians and their contact information 
• List of questions for our healthcare team

New Patient Forms

For your convenience and to save time, we encourage new patients to download, print and complete the new patient forms prior to their first appointment. If not, please arrive approximately 20 minutes early in order to complete these forms in the office.
